In order to maintain an effective flow of information and product shipments for repair or service, the following information would be helpful to have ready when completing the RMA request form. While some may not be required, it may be helpful in completing the process more rapidly.
- Part type of the failed item (mandatory)
- System serial number, if root cause analysis is requested (mandatory)
- Serial number(s) of the failed item(s) (recommended)
- Any customer related references numbers such as order# etc. (recommended)
- Repair cost limit or other additional information (recommended)
